Proof of sponsorship and/or private accommodation

You want to invite someone from abroad who requires a visa for a short term visit. If you want to sponsor this foreign national (guarantee) for cost of living in the Netherlands, medical care and repatriation of this person, you need a proof of sponsorship and/or private accommodation. This form

Proof of sponsorship and/or private accommodation

You need this form when you want to invite people from abroad coming to the Netherlands who require a visa and whom you will provide private accommodation. You also need this form if you want to financially support a foreign national who requires a visa. Clearly specify on the form what is applicable in your case: sponsorship, accommodation or both.

Part 3a: Fill in this part if you only want to provide accommodation.

Fill in this part if you want to sponsor the foreign national financially.

Legalization of your signature and that of your partner

After you have completed the form, your signature should be legalized at city hall.
Note: In case of sponsorship the signature of your spouse or registered partner should be legalized also.

Terms

  1. The sponsor/provider of accommodation must come in person to sign;
  2. In case of sponsorship, your spouse/registered partner needs to come in person also;
  3. You sign in the presence of the registrar at city hall.

Particulars

What if the accommodation provider is not the same person as the sponsor? If the accommodation provider is not the same person as the sponsor, two separate forms must be filled in: one form must be filled in by the accommodation provider and one form by the sponsor and his/her spouse or registered partner. All these people must come in person to sign the forms in the presence of the registrar at city hall.
